Amendments in existing passport
For all the following changes submit form C and the details mentioned with the respective cases.

Change of Marital status
Marriage Certificate duly attested by the Ministry of Foreign Affairs in Pakistan
Change of Name
Notification for change of name in any of the leading Pakistani Newspapers, duly attested by the Ministry of Foreign Affairs, Islamabad required.
Change of Religion
A self written Affidavit duly attested by the concerned authorities
Endorsement of Children
Both parents' passports & permanent residence cards
two passport-sized photos of the child/children
National Identity Card of the parents, if available
Copy of birth certificate where both child's citizenship & parents' names should be mentioned


Processing time and fee

Normal:
Returned after 7 working days
Fee DKK 150
Urgent:
Returned after 2 working days
Fee DKK 300

Embassy does not accept cash payments. Acceptable modes of payment are:

Card (acceptable cards are : All Danish cards having chip)Master card is not acceptable.
Bank transfer/deposit at Danske Bank Account no. 4001 1096 1874 reg nr. 4001. Please attach original bank deposit slip with your application
